Community Participation

Community involvement and socialisation is a very important aspect of a fulfilling lifestyle. As such, we encourage and support our participants to access community-based activities. Our goal is to further develop confidence and build additional social and life skills.

We encourage our participants to be involved in the decisions that affect them. We have a great team who work hard to make sure our participants have access to opportunities to improve their lives. This can be done in a number of ways including:

  • Assistance to get involved in their communities through volunteering opportunities
  • Encouragement of participation in local events, such as local festivals or community days
  • Providing training opportunities into employment or education
  • Providing general Community access for day-to-day shopping, medical appointments, and social outings

Supported Independent Living (SIL)

Empower Care offer support to customers who choose to live independently. We offer up to 24 hour supported independent living services, either in your own home or a home managed by Empower Care. We offer tailored supports that are needs based, and goal orientated. We have flexible programs to cater to all needs in the home. We concentrate our supports on capacity building and skill development.

Our supported living services are designed to make it easier for you to live in your own home. We can provide assistance with meals, laundry, shopping and other daily tasks associated with independent living - so that you have more time for yourself. Support Coordination

Our Support Coordinators are caring, reliable, creative, and up for any challenges that might arise. We will make sure you get a variety of supports, so that you can increase your capacity to maintain relationships, manage service delivery tasks and live more independently. Our support coordinators will work closely with you to identify your needs and help you access your personalised supports.

You can talk to us about any issues that are affecting your day-to-day life and we will help you find the right services that meet your needs. We can also help you find ways to maximise your funding and coordinate other support services and programs. Contact us to find out how we can help you manage your support plan.

Plan Management

Our Plan Managers are the best at what they do. Plan Managers help you manage your money and will keep you informed about your financial situation. They will be responsible for making sure that the money you have set aside for care is spent on the right things according to your support plan. We can also help you find ways of saving money while still getting the support you need.

We will work closely with your family and friends as well as professionals who are involved in your care, like doctors and nurses, to ensure that everyone knows what needs to be done and when it needs to be done. Our Plan Managers know NDIS funding comprehensively and will support you to keep track of your spending and get more out of your funds. Employment

If you're living with an injury, illness or disability, and want to find a job - we can help. As Empower Care grows larger, it provides us an opportunity to offer employment for individuals in our community with a disability.

To be eligible for Disability Employment Services, you must be:

  • with an injury, illness or disability that affects your ability to work
  • an Australian resident aged from at least 15 years to pension age
  • receiving an income support payment, disability support pension or an NDIS participant
  • able to work between 8 to 30 hours per week (with support when required)
  • not studying full time
  • not already working at or above your assessed work capacity
